You must be a Michigan Resident
You must be an organ transplant recipient
You must prove evidence of financial need
You must be 18 years or older
You must be referred by a transplant center social worker or transplant organization in the State of Michigan
You must submit your completed application to your social worker or transplant organization representative
You must provide documents to support your assistance request
Applications will be reviewed and approved within 30 days of submission
Requesting social worker will be notified regarding the decision
Checks will be written and payable to the company/creditor directly on behalf of the patient
Request fits within the non-profit organization’s criteria. Details can be obtained from the online application
Grants will be considered in the order they are received
Grants will be approved in amounts ranging from $100 to $1,000. The grant amount will be based on the need of the applicant
The Grant Committee will determine the amount awarded to the applicant using the following criteria:
The length of time the applicant has been in debt
Will only consider debt incurred from transplant related expenses
If applicant requests more than $1,000, the Committee will eliminate as many outstanding balances as possible; leaving the applicant fewer debtors to contact and pay
