We are now accepting Grant Applications.  Grants will be given in the amounts of $100.00 to $1,000.00.

  

ELIGIBILITY GUIDELINES -

  • You must be a Michigan Resident
  • You must be an organ transplant recipient
  • You must prove evidence of financial need
  • You must be 18 years or older

APPLICATION PROCEDURE AND PROCESS -

  • You must be referred by a transplant center social worker or transplant organization in the State of Michigan
  • You must submit your completed application to your social worker or transplant organization representative
  • You must provide documents to support your assistance request
  • Applications will be reviewed and approved within 30 days of submission
  • Requesting social worker will be notified regarding the decision
  • Checks will be written and payable to the company/creditor directly on behalf of the patient

HOW INDIVIDUAL GRANTS ARE SELECTED -

  • Request fits within the non-profit organization’s criteria. Details can be obtained from the online application
  • Grants will be considered in the order they are received
  • Grants will be approved in amounts ranging from $100 to $2,000. The grant amount will be based on the need of the applicant
  • The Grant Committee will determine the amount awarded to the applicant using the following criteria:
       -  The length of time the applicant has been in debt
       -  Will only consider debt incurred from transplant related expenses
       -  If applicant requests more than $2,000, the Committee will eliminate as many outstanding
          balances as possible, thus leaving the applicant fewer debtors to contact and pay


                        


INSURANCE PREMIUMS ASSISTANCE -

Please submit a completed application to your social worker for consideration. Having health care insurance is important for a successful transplant. Insurance Premiums Assistance is designed to provide temporary financial assistance to non-Medicare covered patients for the cost of their insurance premiums.

  • Grants are for transplant recipient's health care coverage only

DENTAL ASSISTANCE PROGRAM -

Second Chance at Life created a Dental Assistance Program to address and assist with the needs of patients attempting to be listed for an organ transplant. This program was developed with the intent to expedite your dental care thus expediting your listing for a transplant.

In order to be considered for SCAL Dental Assistance, you must work with your social worker for consideration. Also, you must be a pre-transplant patient with dental needs and this must be the FINAL item required to be listed for transplant.
